Christ the Rock Christian Academy

 Enrollment Information

In order to complete enrollment, the following is necessary:

· 2012-2013 Application

· $200.00 Enrollment Fee

· Copy of Birth Certificate

· Current Shot Record

· Copy of Social Security Card

No student is officially enrolled unless all of the above items are presented at the time of enrollment. You will be asked to return at another time with all of the enrollment information if you fail to bring it with you.

Proof of a physical for school-aged students entering school for the first time is required for all students.
